Requesting Appointments:
If you would like to request an appointment, please fill out the following questionnaire – when you click the submit button it will be sent directly to our office .
When we receive your completed questionnaire, we will send you an email to let you know that it has been received.
Dr. Toups personally reviews all applications weekly, and you will receive an email from us as soon as possible to let you know if we are able to accommodate you for an appointment.
